Who should use this form?

Those persons not having a consultant, laboratory or distributor serving their area and are interested in begining to use VitisLabs Online Laboratory Notebook. If you are interested in obtaining your personal contact we will help locate one for you. Begin the registration process by filling out the form below.

To better serve you, Vitis Labs collects customer information for internal uses only. Fo r more details see our Privacy Policy. If you are a returning customer you must use Member Logon.

Each member (consultant or distributor) of the VitisLabs group creates their own custom Online Laboratory Notebook that fits with the services each member provides. This notebook has been customized for Vitis Consulting.  

Registration will incur a setup cost of $YY for this notebook. There is an additional annual subscription fee that is dependent on the number of analyses that are included in your notebook and how many analyses you conduct per year.  Our basic annual subscription fee is $XX, which includes sufficient space and analyses for most small to medium sized wineries.  Registration obligates the user only to the setup charge. The annual subscription fee will not be charged until confirmation of services required and is determined by your consultant.

After the information has been entered in the fields on the left, select the "continue" button below. A username and passoword will be created and sent to the email address used in the form. Please keep this password available for subsequent returns to Vitis Research. At any time if the information in these  fields needs to be updated you may select this link  here or the equivalent link at Members Logon. Enter your username and passoword into the field presented and this will open an update form in which you can update your records.

After completion of the update form you will automatically be returned to a page such as this where the new information will have been automatically entered into these fields. Verify that the information is correct and click the continue button to advance to the next screen.

Please remember, if customer name and or address is different than the name or address on the credit card, please change the credit card information fields to reflect the credit card holder name and address. This error is the most common reason for a decline during the credit card transaction. All credit card information is sent via secure transactions.
